Coordinated Family Supports (CFS)
We’re excited to announce the launch of a new program at Oaks of Hebron,Coordinated Family Supports (CFS). This service is designed to assist adults with developmental disabilities who are 18 years or older and continue to live in the family home.
The goal of CFS is to help individuals and their families access the tools and resources they need to thrive together. Services are person-centered and tailored to meet the unique needs of each individual and their family situation.
CFS services include:
- Supporting skill development in the home
- Coordinating and securing needed services, including assistance with accessing “generic services”
- Helping individuals engage with their communities
- Scheduling important services, such as medical appointments
- Creating reliable backup plans for care and support
- Exploring transportation options
- Planning for the future, especially for individuals with aging caregivers
