Supported Living Services and Independent Living Services for Adults with Developmental Disabilities

Servicing Sonoma County, Supported Living Services (SLS) and Independent Living Services (ILS) are two distinct programs designed to empower adults with developmental disabilities to thrive in their communities. With a focus on choice, dignity, and person-centered planning, these services offer the support and skill building needed to achieve greater independence.

Supported Living Services (SLS)

SLS staff assists individuals who live in their own homes with ongoing support tailored to their individual needs. From personal care to managing household tasks and medication, SLS ensures that participants are safe, comfortable, and in control of their daily lives. The focus of SLS is to provide the appropriate level of support while encouraging independence.

SLS support can include assistance with daily activities such as:

  • Personal Care: Bathing, dressing, grooming, and hygiene support.
  • Household Management: Meal preparation, shopping, housekeeping, and budgeting.
  • Health and Medication: Assistance with medication management, medical appointments, and health-related decisions.
  • Community Involvement: Encouraging participation in social, recreational, and community activities to build relationships and enhance quality of life.

Independent Living Services (ILS)

Through training and support from a one-to-one Community Support Facilitator (CSF), participants gain confidence and the ability to navigate everyday challenges. ILS support empowers individuals to take control of their lives, make their own decisions, and reach their full potential whether they are living independently or with others.

Key areas of ILS include:

  • Skill Development: Training in areas such as personal care, household chores, shopping, cooking, and using public transportation.
  • Problem-Solving: Assistance with learning how to address challenges that may arise in daily living, including safety and decision-making skills.
  • Advocacy and Self-Empowerment: Encouraging individuals to advocate for themselves in medical, social, and legal matters, while building confidence in navigating systems.
  • Money Management & Budgeting: Assistance with creating and maintaining a budget that meets the client’s individual needs.
